You should try setting the Camera Mode to AUTO:
Please note that the format received from the camera differs significantly from what the Intensity Pro 4K is configured to output. It appears that the camera is recording in interlaced (i), while the output is set to progressive (p):
In most scenarios, it's best to have all components (camera input, Aximmetry, and output) configured to the same format, including resolution, progressive/interlaced, and frame rate.
