If you add in your playlist an action to toggle a button that has an icon assigned to it, it will not work. Once you change that to text, it works fine.

I couldn't reproduce it. Was it a logical, trigger, or integer button? Actually, a picture of the pin collector could help a lot.
Note that these two Action type works with control board buttons: And Set pin value works with pins.
Also, if you have a button with the same name. Then the first one will be triggered. Like here instead of the last pin (#Smooth), the first one will be triggered (#Value,Backwards,Smooth) cause it has a Smooth button too:
